Thank you for your interest in our Treatment Provider Database. If you are a mental health professional who treats OCD and/or OC related disorders and have an active license to practice, please select one of the two options below.
New Provider Listing
- Provide us with a copy of your existing mental health license. If you have not recently sent us a copy, please do one of the following:
- Email a copy of your license to Stephanie Cogen, Assistant Program Director, at scogen@ocfoundation.org
- Fax a copy of your license to 617-973-5803
- Mail a copy of your license to:
